Register your family for Lakewood Congregational Church’s Faith Formation program! Use this form to share emergency contacts, allergies, and other important information for infants through Grade 12 students. All children must have this form on file, even if a parent or guardian will be at church the whole time.
Liability and Medical Form
This form is needed for various participants at Lakewood Congregational Church. Students participating in events outside of Sunday morning faith formation will need to have this filled out annually. You will be notified if you need to complete it.
